CANCELLATION POLICY
FADE AND FORM SKIN AESTHETICS Cancellation & Appointment Policy At Fade and Form Skin Aesthetics, appointments are reserved specifically for each client. When an appointment is booked, that time is removed from our schedule and made unavailable to other clients. CANCELLATION & RESCHEDULING Clients must provide at least 24 hours’ notice to cancel or reschedule an appointment. Cancellations or appointment changes made less than 24 hours before the scheduled appointment time will result in a cancellation fee equal to 50% of the scheduled service price. NO-SHOWS Failure to appear for a scheduled appointment without notice will be considered a no-show and will result in a fee of 100% of the scheduled service price. Clients who arrive more than 15 minutes late may be required to reschedule if the remaining appointment time is insufficient to safely and appropriately perform the scheduled service. If the appointment must be forfeited because of the late arrival, the applicable cancellation/no-show fee may apply. SAME-DAY CANCELLATIONS Same-day cancellations are considered cancellations with less than 24 hours’ notice and are subject to the 50% cancellation fee. EMERGENCIES & SPECIAL CIRCUMSTANCES We understand that unexpected circumstances happen. Cancellation fees may be waived at the sole discretion of Fade and Form Skin Aesthetics for documented emergencies or circumstances determined by the business to warrant an exception. Repeated late cancellations or no-shows may result in a client being required to pay a non-refundable deposit or prepayment before future appointments can be scheduled.
